Lost, stolen or damaged tag decal or plate? How to replace it

Replace a decal or plate online or in person, the fees and what to bring.

Updated by Eastside Tag Agency

Decals get peeled off, plates get bent, and sometimes both just disappear. Oklahoma makes replacing them straightforward.

Replace it online

Service Oklahoma lets you order a replacement without coming in:

Both open the State's Navigate site with our office already selected, and the replacement is mailed to you.

Or replace it in person

Come by 1914 S Garnett Rd, Tulsa, OK 74128 with your driver license and proof of Oklahoma insurance to replace it at our office. You'll sign Form 797-D, the Affidavit for Replacement License Plate/Decal, in front of a notary, and we have one here. The State's replacement fee applies; it's listed below.

The form is also on our forms page if you'd rather mail it in, notarized.

What it costs

These are the State's fees from Form 797-D:

What you're replacingReplacement fee
Registration decal$9
Decal and license plate$9
Boat or outboard motor decal$2.25

Motor vehicles also pay the $1.50 insurance fee, and there's a mailing fee if the replacement is sent to you: $1.73 for a decal, or $6.60 for a decal and plate.

Want the new plate design?

If your plate is damaged anyway, it's a good time to switch to the new red Oklahoma plate. Outside of your yearly renewal, the plate replacement fee is $9.

If your plate was taken for no insurance

A plate seized by law enforcement for lack of insurance can't be replaced until the fines are paid and the county sheriff issues a release. Bring the release with you.
